• Collect only objective data.
• Clearly define your data elements.
• Document detailed instructions for drilling down to each data element.
• Limit each data element to only one unit of information. Don't lump multiple questions into one data element.
• Make sure the time period is clearly stated, especially if you are looking for something during a specific time period.
• Provide training for all auditors.
• Pilot test the data collection tool. If your auditors are confused as to how to interpret and answer questions, provide more clarity.
