Streamline your orders requests with this handy form. Getting the orders you need to take credit for the best practice interventions in M2250 -- Plan of Care Synopsis doesn't have to be difficult. Try using a fax template like the one below from Judy Adams, RN, BSN, HCS-D, COS-C with Adams Home Care Consulting in Chapel Hill, N.C. Simply fill out the checklist with your recommendations and fax the form to the physician for his signed agreement to the proposed plan. Provided the physician responds in time, you can use this information to complete M2250 (see related story, p. 74).
