Try the SBAR Approach for Cross-Provider Communication

Communication always flows better when you're prepared. To that end, the Centers for Medicare & Medicaid Services suggests taking the SBAR approach in discussing patient care with other healthcare providers. This technique involves identifying four elements important to the information you plan to share: the Situation, the Background, the Assessment, and the Recommendations. This structure can be especially helpful for communication with other healthcare team members. Here's a sample SBAR worksheet provided by CMS:
