Keep documentation for the part of the cervix where the lesion is located.

You need to be careful about reporting the diagnosis of carcinoma in situ in the female genital tract as ICD-10-CM may not offer a direct match for every code. Learn more about how to report these lesions in ICD-10-CM.

Specify Parts of Cervix for Carcinoma In Situ

In ICD-9-CM, you have only one single code for carcinoma in situ of the cervix, i.e. 233.1 (Carcinoma in situ of cervix uteri). 

ICD-10-CM: You will need to ensure your oncologist specifies where in the cervix was carcinoma in situ found. For carcinoma in situ of the endocervix and exocervix, you report codes D06.0 (Carcinoma in situ of endocervix) and D06.1 (Carcinoma in situ of exocervix), respectively. For carcinoma in site in any other part of the cervix, you report code D06.7 (Carcinoma in situ of other parts of cervix).

When your oncologist does not specify any part of the cervix in the documentation for diagnosis of carcinoma in situ, you submit code D06.9 (Carcinoma in situ of cervix, unspecified).

Watch for Descriptor of Endometrial Carcinoma In Situ 

ICD-10-CM offers a one-to-one match for carcinoma in situ in the uterus. However, there is a change in code descriptor. The ICD-9-CM code 233.2 (Carcinoma in situ of other and unspecified parts of uterus) reads ‘other and unspecified parts’ of the uterus. However, the ICD-10-CM code D07.0 (Carcinoma in situ of endometrium) specifically mentions ‘endometrium’, which is the typical site of carcinoma in situ in the uterus. 

Other Parts of Female Genitalia Have Direct Matching Code

For carcinoma in situ for other parts of female genital tract such as vagina and vulva, ICD-10-CM offers a direct one-to-one match with same descriptors. The same applies to codes that you report when the physician does not specify the location of the carcinoma in situ in the female genital tract. These are listed in table 1.
