Answer: The proper code is 77280 (Therapeutic radiology simulation-aided field setting; simple), according to CPT guidelines.

Don't let the opposing ports fool you into choosing 77285 (- intermediate). CPT defines simple as -simulation of a single treatment area with either a single port or parallel opposed ports. Simple or no blocking.-

Intermediate, on the other hand, requires -simulation of three or more converging ports, two separate treatment areas, multiple blocks.-
