Remember That an IOL Is an Implant

Question: Is there a separate code only for intraocular lens removal (IOL) or should we use an unlisted code?

Answer: You shouldn’t use an unlisted code because the CPT® code book includes two codes related to the removal of an IOL:

  • 65920 (Removal of implanted material, anterior segment of eye)
  • 67121 (Removal of implanted material, posterior segment; intraocular)

The difference between the two codes depends on where the IOL procedure is occurring. Use 65920 when the IOL is located in the anterior segment of the eye. This means that the lens was implanted in front of the iris. Ophthalmologists will insert the lens here when the capsule behind the lens is damaged or weak.

Assign 67121 when the provider removes the IOL from the posterior segment of the eye. The posterior chamber is located behind the iris, and the capsule and capsular bag provide support for the lens.

Coding tip: The helpful thing to remember for an IOL removal procedure is that the material being removed is an implant. When a code descriptor states, “removal of lens material,” that is referring to the natural lens of the eye, whereas “implanted material,” as in the case of 65920 and 67121, is an IOL.
