Follow the chart to get the right CPT code for almost every combination of eye muscles

With several different muscles in each eye - each with its own code - coding for strabismus surgery can get tricky. Our chart above can help you arrive at the right code fast.

To use the chart, find the muscles operated on in the "right eye" and "left eye" columns. Draw a line over from the "left eye" column and down from the "right eye" column. The intersection of those lines contains the codes you should report.

Example: The ophthalmologist operated on one vertical muscle in the right eye and two horizontal muscles in the left eye. Those lines meet at "67312-LT, 67314-51-RT," which are the codes you should report.

Remember: Report add-on codes 67320-67340 separately in conjunction with primary procedure codes 67311-67318.
