Question: How should we code for the removal of a dislocated intraocular lens (IOL), removal of some vitreous and insertion of a secondary IOL? Florida Subscriber Answer: You should report 66986 (Exchange of intraocular lens) for the removal of the dislocated IOL and implantation of another IOL. In addition you should bill 67010 (Removal of vitreous, anterior approach [open sky technique or limbal incision]; subtotal removal with mechanical vitrectomy) for the subtotal vitrectomy.
