Reader Question:

Periocular Injections

Question: What code is used for a periocular injection?

Brenda Purvis, Valley Eye Center
Van Nuys, CA

Answer: There is no specific code for a periocular injection, which is done in the muscle, often as an anesthetic. A periocular injection doesnt have the same risk as a retrobulbar injection, but retrobulbar (67500) or Tenons capsule (67515) are the closest. Whichever you use, you would have to use a -22 modifier to indicate an unusual procedure; or, alternatively to use an unlisted procedure code.

Note: Both 67500 and 67515 are starred procedures. That means that the codes only include the surgical procedure; you should charge for pre- and postoperative visits as well.
