Reader Question:

Understand Modifier -32

Question: When should I use modifier -32 and how does this modifier affect payment?

Florida Subscriber Answer: Append modifier -32 (Mandated services) to consultation codes when your physician performs a confirmatory consult, that is, a consultation generated by a patient, a patient's family, a physician or even an insurance carrier that seeks a second opinion. The only confirmatory consult codes that require you to append modifier -32 are those brought on by the carrier.

Modifier -32 does not change the fee for the service to which it is appended; however, you should use modifier -32 as an informational modifier that indicates that the carrier requests a confirmatory consult before agreeing to cover the service.   - Clinical and coding expertise for You Be the Coder and Reader Questions provided by Raequell Duran, president, Practice Solutions, Santa Barbara, Calif.
