Question: My ophthalmologist returned to the OR this morning to repair a wound leak following an IOL insertion she did last Thursday. I'm having a hard time finding a code for this. What code and modifier do I need?

Pennsylvania Subscriber Answer: Report 66250 (Revision or repair of operative wound of anterior segment, any type, early or late, major or minor procedure) for the wound leak repair. Append modifier -78 (Return to the operating room for a related procedure during the postoperative period) to show that the wound leak is a complication of the original IOL insertion.
