Supervision Levels for Ophthalmological Diagnostic Tests

 Keep the levels straight - and elevate your documentation to the next level

CMS has determined that certain diagnostic tests under the "Special Ophthalmological Services" heading in the CPT manual require either general or direct supervision when a nonphysician practitioner performs them. Use this key to help keep your documentation on the level.

Level One: General Supervision

Physician provides overall direction and control but does not need to be present.

  • 92060 - Sensorimotor examination with multiple measurements of ocular deviation with interpretation and report (separate procedure)
  • 92065 - Orthoptic and/or pleoptic training, with continuing medical direction and evaluation
  • 92081 - Visual field examination, unilateral or bilateral, with interpretation and report; limited examination
  • 92082 - ... intermediate examination
  • 92083 - ... extended examination
  • 92135 - Scanning computerized ophthalmic diagnostic imaging with interpretation and report, unilateral
  • 92250 - Fundus photography with interpretation and report.

    Level Two: Direct Supervision

     Physician must be present in the office suite and immediately available to assist in and direct the procedure.

  • 92235 - Fluorescein angiography (includes multiframe imaging) with interpretation and report
  • 92240 - Indocyanine-green angiography (includes multiframe imaging) with interpretation and report.
