Answer: Code 92250 (Fundus photography with interpretation and report) is not bundled with an eye exam (92002-92014) or consultation (99241-99245) -- Medicare's Correct Coding Initiative (CCI) is quite clear on this. This means that you can bill for an office visit or consultation on the same day that you bill for a fundus photograph. The code used for the visit will be based on what was included in the exam and on the patient's complaints.
In addition to the fundus photos, take note that the accompanying chart should also contain:
• an order for the test
• the patient's name and date of the test
• indications for the test
• an interpretation of the results with a report
• the optometrist's signature.
