Question: I am going to see a terminally ill patient at home tomorrow. She's a new patient, and she has Medicare. Is there a home-visit equivalent to evaluation and management code 99204? Answer: For a new patient, you can bill a code from the 99341-99345 series (Home visit for the evaluation and management of a new patient ...). For an established patient, use a code from the 99347-99350 series (Home visit for the evaluation and management of an established patient ...). Be sure to use place of service (POS) code 12 (Location, other than a hospital or other facility, where the patient receives care in a private residence).
