Submit One Cast Supply Code Unit Per Cast

Question: Should we use supply code Q4010 as one unit, or can we report multiple units?

Answer: You should bill all the cast supply codes as one unit per each cast your orthopedist applies.

Tip: Don’t miss out on application pay by checking if the cast application encounter involves a surgical procedure. You can bill Q4010 in addition to the cast application (such as 29085, Application, cast; hand and lower forearm [gauntlet]) provided your orthopedist is not performing a surgical procedure on the same date of service. If your orthopedist did perform a surgical procedure, payers would bundle the cast application into the procedure, as well as injections. You can still, however, separately bill the materials (Q4010).
