CMS Announces Demo Project For High-Cost Benes

The Centers for Medicare & Medicare Services wants to help the costliest Medicare patients obtain the care they need. A new demonstration project will study different care management models for the highest cost patients in traditional Medicare. These models could include intensive case management, greater access to providers, chronic care programs, restructured practices and more flexibility in the settings where beneficiaries receive care.
