Include BMI Calculation in E/M

Question: We have been calculating our patients’ body mass index (BMI) when we perform well visits. How do we bill for the BMI calculation?

Answer:
Calculating BMI is included in the office visit—specifically the physical exam portion of the visit. Although the American Academy of Pediatrics includes a recommendation of BMI screening in its Bright Futures guidelines, that doesn’t mean you’ll collect a separate fee for it.

However, the added BMIrecommendation starting at age two means you may report an additional ICD-9 code. Use the BMI percentile to choose the correct V85.5x code. For instance, you would represent a 3-year-old girl’s 10 percent BMI with V85.52 (Body Mass Index, pediatric, 5th percentile to less than 85th percentile for age). You would include the examination in the preventive medicine service.
