Question: I saw a mother of twins who requested advice on dealing with both babies' needs, as well as additional time off from work. She asked me to complete a Family and Medical Leave Act (FMLA) form. How should I code for this added work?

Answer: For the form's completion, you may assign 99080 (Special reports such as insurance forms, more than the information conveyed in the usual medical communications or standard reporting form) in addition to the office visit code (99201-99215, Office or other outpatient visit ...).
 
Because FMLA forms require a lot of work, about half of pediatric practices charge for completing these forms, estimates one pediatric coding expert.

Consider 99080 when you complete forms that require more information than you usually provide in a standard report, such as school and sports forms.
