Stay Unshaken When Coding Tremors

Question: I hope you can shine some light on this as we have another dilemma today as to whether we should use Z86.79 (disease of circulatory system) or if we should use G25.2 (tremors)?

Answer: Judy Adams, RN, BSN, HCS-D, AHIMA approved ICD-10-CM Trainer, with Adams Home Care Consulting in Asheville, N.C. says: “Z86.79 is a nonspecific aftercare code and tremors is a specific medical condition. If the care is directed at treating the tremors, the most specific code that reflects the patient’s medical condition should be used.  If the tremors are a complication of surgery, an injury or part of another medical condition, the underlying cause should be coded first.”

However, you need to keep in mind that official guidance states diseases of the nervous system (G00-G99) Excludes2; that is, they can’t be used in certain conditions originating in the perinatal period (P04-P96).
