Median Branch Nerve Injection

Question: Our physiatrist performs median branch nerve injections on single and multiple levels. What code is correct for these procedures?

Answer: It sounds as though you are referring to facet joint nerve injections and, therefore, the correct code for one level would be 64475 (Injection, anesthetic agent and/or steroid, paravertebral fact joint or facet joint nerve; lumbar or sacral, single level).

If the physiatrist injects multiple levels, report the add-on code +64476 (... each additional level [list separately in addition to code for primary procedure]) for each subsequent level. Therefore, a claim for three lumbar or sacral levels would appear as follows:

64475 (representing the first level)
64476 (for the second level)
64476 (for the third level).
