Be Careful of Inclusive Services

Question: My podiatrist performed a radiologic ankle arthrography with supervision and interpretation. I reported 73615 and +77002 on my claim, but the +77002 was denied. Can you explain why?

Answer: You cannot report +77002 (Fluoroscopic guidance for needle placement (eg, biopsy, aspiration, injection, localization device) (List separately in addition to code for primary procedure)) with 73615 (Radiologic examination, ankle, arthrography, radiological supervision and interpretation), according to the CPT® guidelines.

In the text below the code descriptor for +77002 in the CPT® manual, you will find this statement: “+77002 is included in all arthrography radiological supervision and interpretation codes.” So, since +77002 is considered an inclusive service of 73615, you cannot report a separate code for this procedure.
