Use This Log To Track Your Denials

Keeping tabs on your practice's denied claims is an essential tool for saving yourself time and money on the back end, while collecting more reimbursement on the front end to boot. Use this tool to track your denials, provided by Physician Practice Management, Inc. in Atlanta. Instructions:
1. Complete one row for each denial your practice has received from an insurance company.
2. Record the denial amount under the appropriate category; if the error occurred at registration, for example, record the amount under that column header.
3. Use your judgment about the categories - and the reasons that they fall under each; the key is not to place blame, but to identify root causes and where to go to fix the problem.
4. Write the specific reason for the denial under "Reason."
Note: We suggest one page per day or week, depending on volume [of denials]. Review each month to identify problem areas and trends.
