Question: A patient presented to our clinic complaining of cough, shortness of breath, chest pain, and nausea. During the patient history, the pulmonologist asked if the patient smokes cigarettes. The patient attested that they vape from time to time in an effort to quit smoking. After testing and a physical examination, the physician diagnoses the patient with EVALI.

Answer: E-cigarette, or vaping product, use associated lung injury (EVALI) is a serious medical condition where substances found in e-cigarettes and vaping products cause damage to the patient’s lungs. You’ll assign U07.0 (Vaping-related disorder) to report an EVALI diagnosis.

Code U07.0 went into effect April 1, 2020, and is used to report several conditions caused by vaping. Additional diagnoses reported by U07.0 include:

  • Dabbing related lung damage
  • Dabbing related lung injury
  • Electronic cigarette related lung damage
  • Electronic cigarette related lung injury

A “Use additional code” note under U07.0 also instructs you to identify any of the following manifestations:

  • R10.84 (Generalized abdominal pain)
  • R19.7 (Diarrhea, unspecified)
  • R63.4 (Abnormal weight loss)
  • J69.1 (Pneumonitis due to inhalation of oils and essences) or lipoid pneumonia
  • J70.4 (Drug-induced interstitial lung disorders, unspecified)
  • J80 (Acute respiratory distress syndrome).
