Be sure you've got revised MLN article MM5060 You don't want to miss this revision to MLN article MM5060, "Additional Requirements Necessary to Implement the Revised Health Insurance Claim Form CMS-1500."
The original article instructed you to use the billing provider's NPI on certain fields of the new CMS-1500 form. Instead, the article should read, use the "appropriate NPI" on the following items:
• 24J (replacing item 24K, Form CMS-1500 [12-90])
• 17B (replacing item 17 or 17A, Form CMS- 1500 [12-90])
• 32a (replacing item 32, Form CMS-1500 [12-90])
• 33a (replacing item 33, Form CMS-1500 [12-90]). You can download the revised article at
