Beware Trying to Report 77014 and 77295 Together

Question: Our radiologist provided CT guidance for placement of radiotherapy fields. How can we code for this service?

Answer: You submit code 77014 (Computed tomography guidance for placement of radiation therapy fields) for CT guidance to help place the radiotherapy fields.

However, make sure you do not submit code 77014 with 77295 (3-dimensional radiotherapy plan, including dose-volume histograms). Code 77014 is a column 2 code for 77295 and these codes cannot be billed together in any circumstances.

According to CPT® explanatory notes, “3-dimensional” in this case means “computer-generated 3-D reconstruction of tumor volume and surrounding critical normal tissue structures from direct CT scans and/or MRI data in preparation for non-coplanar or coplanar therapy.” The CT guidance explained in 77014 is thus part of 77295 when performed on the same date of service.
