Reader Questions:

Arthrogram,Then MRI

Question: How should I code an arthrogram followed by MRI? I am wondering if we should code for the arthrogram and then for an MRI with contrast. Florida Subscriber Answer: If a full arthrogram is provided in advance of a contrast MRI, then the anatomically correct procedure code for the injection is reported once. Just be sure that the imaging code you chose for the arthrogram is appropriate. The RS&I code would be the anatomically correct RS&I code for the arthrogram.

Choose the MRI code by locating the most anatomically specific MRI joint code (with contrast) for the body site in question. If the radiologist only performs guidance fluoroscopy (with or without confirmatory films) but does not perform a full arthrogram, then you should report only 76003 (Fluoroscopic guidance for needle placement) in conjunction with the anatomically correct procedure code and an MRI with contrast.
