Mesh Removal Code Depends on Reason for Condition

Question: The surgeon used an open technique to remove a large amount of mesh material that had eroded into the patient’s bladder. He wants to charge 51865. Is this correct?

Answer: A clear answer is not apparent based on the limited information you share. However, if the mesh was from a previous sling operation for stress incontinence, then you should report 57287 (Removal or revision of sling for stress incontinence [e.g., fascia or synthetic]). CPT® code 51865 (Cystorrhaphy, suture of bladder wound, injury or rupture; complicated) is not correct for the clinical scenario you describe. That’s because you should only report 51865 for the repair of a rupture or injury of the bladder, not for the removal of a displaced mesh. Append ICD-10 diagnostic code T83.711A (Erosion of implanted vaginal mesh and other prosthetic material to surrounding organ or tissue, initial encounter).
