Submit A4648 for Each Marker, Not Each Pack

Question: My urologist places markers that come in a kit of three markers in a package and uses them in a cyberknife procedure in which four markers are placed. Can I bill for two kits? I use A4648 which just states “tissue marker, implantable, any type, each.” What’s the correct way to code and account for all the markers?

Answer: In this situation, “each” means each individual tissue marker, based on the descriptor for HCPCS code A4648 (Tissue marker, implantable, any type, each). If the provider used four markers, you report four individual markers regardless of the pack size. Bill through your local carrier and include the purchase invoice for the markers.
