Therefore, you should use the unlisted ultrasound procedure code, 76999 (Unlisted ultrasound procedure [e.g., diagnostic, interventional]). Your urologist must prepare a full report and you should submit that report with a covering letter stating what the physician did and why, the time and effort spent,as well as the equipment and supplies he used.
