50605: To Add or Not to Add to Pyeloplasty Coding?

Question: How should I report "dismembered pyeloplasty with right nephrostomy and placement of double-J stent"? Tennessee Subscriber Answer: You should report CPT 50400 (Pyeloplasty [Foley Y-pyeloplasty], plastic operation on renal pelvis, with or without plastic operation on ureter, nephropexy, nephrostomy, pyelostomy, or ureteral splinting; simple) for this procedure. Watch out: The Correct Coding Initiative ( CCI Edits ) bundles 50605 (Ureterotomy for insertion of indwelling stent, all types) into the pyeloplasty and you should not bill that code in addition to the 50400.
