Procreative Management in ICD-10

Question: We see male patients for possible infertility. At the new patient visit, we do not have confirmation of infertility. We have been using Z31.89 in ICD-10 for procreative management; however, we wonder if the procreative management codes are acceptable as principal diagnostic codes. Where should we be looking for a principal diagnosis on these patients?

Answer: The new ICD-10 Z codes replaced the ICD-9 V codes on October 1, 2015. Unlike the ICD-9 V codes, you may use the Z codes as either principle/first listed codes or as secondary codes, depending on the circumstances of the encounter, according to AMA Principles of ICD-10-CM Coding Manual by Deborah J. Grider (page 250).

However, certain Z codes may only be first listed, and this is so indicated in the ICD-10 manual by the symbol PDx. Based on the lack of PDx symbol in the ICD-10 manual notations, it appears that you may use Z31.89 (Encounter for other procreative management) as either a primary or secondary code.
