Wiki 99212 & 99214 billed together on same day?

Just curious if anyone has ever billed 99212 and 99214 for the same visit on the same day? Do both E&M's get paid and are there modifiers involved?

Can you provide more context as to why there would be the need to bill two E/M would be billed on the same visit same day?
 
Exactly, doesn't make sense at all. We received records from another office and on the visit for an ear infection. 99212 was listed as a nurse visit and then 99214 was listed underneath? Just wanted to make sure that this is not correct.

No, if the patient sees a physician/APP the nurse visit is not separately billable. The only other billable service would be something that has it's own CPT code for example an injection/infusion that is within the nurse's scope of practice in your state provided under direct supervision and then it's billed under the physician/APP supervising.
