CIgna approves more than 5 visits but ASH denies.

I'm not familar with Cigna but dealing with ASH its typically ASH that does the auth, whether needs additional review of just submission of the rx to ASH depended on plan.
 
Tiers

ASH uses a tier system. From the sounds of it, you are in Tier 1. As you successfully submit claims to ASH and follow their guidelines, they will move you up through the tiers. If you log in to ASHLink (if you use it), in the upper right hand corner it lists your clinic/address/and Tier. If you click on the tier, it will give you information about how to move up.
We just played their game. We are now in tier 6 and, assuming we continue to play by their rules, will be able to treat patients within their health plan limits.
