Wiki Sepsis in outpatient

Patient was hospitalized following a fall and was determined to have sepsis. She is now being seen by primary care for a hospital follow-up. Pt is still on antibiotics for the sepsis. Would the sepsis be coded as still active or would this be considered as historical at this point?
Can a sepsis diagnosis even be used in an outpatient office setting, or is it like a stroke and should be coded as historical after discharge?
Appreciate any advice!

Treatment = active diagnosis
Completed treatment = history (does not apply to chronic diseases)

Thanks. I guess I'm also concerned about whether this diagnosis is allowed in outpatient billing, The only thing I pulled up in an internet search about coding sepsis in outpatient described pretty much this same scenario and said it was inappropriate. It wasn't a source I am familiar with, so I don't put a lot of stock in it. I want to tread lightly, since I know sepsis is a good target for an audit.
