When would a physician use Decision not-to-resuscitate or to-de-escalate care because of poor prognosis? Please give me an example.

Someone with a multi-trauma, TBI, massive wounds and not expected to survive, living will is reviewed and family decides comfort care only/DNR on file.
Individual has end-stage disease (like cancer for example) and does not want any further treatment, testing or other measures other than hospice or comfort measures, decision made in oncology office.

It's pretty straightforward and would be fairly easy to find in documentation.
