Question: Should I code the presence of a ramus intermedius as an anomaly?

Answer:
You should not code the ramus intermedius (RI) as an anomaly using 746.85 (Coronary artery anomaly congenital) (ICD-10-CM: Q24.5, Malformation of coronary vessels). Under CPT® and Correct Coding Initiative (CCI) guidelines, the RI is considered a major coronary artery. The RI is a normal variant occurring in about 10 to 30 percent of the population. Anomalies are often defined as occurring in less than 1 percent of the population.
