Question: Will we be able to report +92978 with the new coronary revascularization codes?

Answer: Yes. An instructional note with +92978 (Intravascular ultrasound [coronary vessel or graft] during diagnostic evaluation and/or therapeutic intervention including imaging supervision, interpretation and report; initial vessel [List separately in addition to code for primary procedure]) adds the following new codes for coronary revascularization services as acceptable primary codes: 92920 (angioplasty), 92924 (atherectomy), 92928 (stent), 92933 (atherectomy with stent), 92937 (graft revascularization), 92941 (revascularization during acute myocardial infarction), and 92943 (revascularization of chronic total occlusion).

The 2013 note also adds two previously existing codes to the primary code list:

• +93563, Injection procedure during cardiac catheterization including imaging supervision, interpretation, and report; for selective coronary angiography during congenital heart catheterization (List separately in addition to code for primary procedure)
• +93564, … for selective opacification of aortocoronary venous or arterial bypass graft(s) (e.g., aortocoronary saphenous vein, free radial artery, or free mammary artery graft) to one or more coronary arteries and in situ arterial conduits (e.g., internal mammary), whether native or used for bypass to one or more coronary arteries during congenital heart catheterization, when performed (List separately in addition to code for primary procedure).
