Don't Mix Up 92975 With 92977

Question: The cardiologist used a catheter to infuse urokinase into the affected coronary artery of a patient who has a myocardial infarction (MI). The urokinase broke and dissolved the thrombus. Should I report 92975 or 92977 for this procedure?

Answer: For this procedure, you would report 92975 (Thrombolysis, coronary; by intracoronary infusion, including selective coronary angiography), not 92977 (Thrombolysis, coronary; by intravenous infusion).

Although both codes are used for coronary thrombolysis, you will see that the difference between them lies in the approach. Report 92975 when the cardiologist employs intracoronary infusion and report 92977 when the cardiologist employs intravenous infusion.
