Keep Biventricular Upgrade Coding Simple

Question: The cardiologist removed an existing dual-chamber pacemaker generator, replaced it with a new biventricular pacemaker generator (so three ports), and added a left ventricular lead. I want to be sure I found all the codes. Which ones are correct?

Answer: You should report the generator work using single code 33229 (Removal of permanent pacemaker pulse generator with replacement of pacemaker pulse generator; multiple lead system).

Then add left ventricular lead code +33225 (Insertion of pacing electrode, cardiac venous system, for left ventricular pacing, at time of insertion of implantable defibrillator or pacemaker pulse generator [e.g., for upgrade to dual chamber system] [List separately in addition to code for primary procedure]).
