Reader Question:

LC, LD, RC Information Found in Payer Manual

Question: I searched Interventional Cardiology LCD L30283 regarding 92980-92984, but it doesn't indicate modifier requirements for location of the intervention. Where can I find this information?

Answer: The LCD you reference is from Trailblazer, which offers a Modifier Manual at www.trailblazerhealth.com/Publications/Training%20Manual/Modifiers.pdf.

Page 5 of the modifier manual has a "Coronary Artery" section. It states:

  • LC Left circumflex coronary artery
  • LD Left anterior descending coronary artery
  • RC Right coronary artery

Use these modifiers to indicate the specific vessel involved in the procedure; they are only valid for CPT® codes 92980, 92981, 92982, 92984, 92995 and 92996.

These codes include:

  • 92980-+92981, Transcatheter placement of an intracoronary stent(s), percutaneous, with or without other therapeutic intervention, any method ...
  • 92982, +92984, Percutaneous transluminal coronary balloon angioplasty ...
  • 92995-+92996, Percutaneous transluminal coronary atherectomy, by mechanical or other method, with or without balloon angioplasty ...
