Question: The provider forgot to enter the date of the initiation of the chiropractic treatment in the item 14 of Form CMS-1500. Would this affect reimbursement?

Answer: Such a claim is unfortunately bound to be denied because this data is required to make the payment. As per the Claims Adjustment Reason Code 16, “Claims/service lacks information which is needed for adjudication.”
