Question: What are the facts to keep in mind when filling boxes 14 and 15 for a new Medicare patient?

Answer: You must always fill box 14, as per Medicare requirements. Enter the date when the patient presented to the provider with his chief current complaint, for which he sought medical care. Medicare requires box 15 to be left blank.

However, other insurers require box 14 to be filled with the date of the incident.
