CMS will calculate the data based on HIS items in the tool’s six categories:
Section A: Administrative Information
Section F: Preferences
Section I: Active Diagnoses
Section J: Health Conditions (Pain and Dyspnea)
Section N: Medications
Section Z: Record Administration.
