Is your facility using these acronyms or symbols on the JCAHO 'Do Not Use' list? 

The Joint Commission on Accreditation of Healthcare Organizations has issued a list of dangerous abbreviations, acronyms and symbols that providers should not use in any resident documentation.
        
Beginning Jan. 1, 2004, the following items must be included on each accredited organization's "Do not use" list in order to comply with the 2004 patient safety goals.
