Question: Should I report a J code for Tensilon in addition to code 95857? Illinois Subscriber Answer: When you submit 95857 (Tensilon test for myasthenia gravis), you should include a J code. Report J3490 (Unclassified drugs). Extra help: Some payers include information on their Web sites about medications for J3490. The West Virginia Department of Health and Human Services, for example, has information stating that your claim for Tensilon should include either diagnosis 358.00 (Myasthenia gravis without [acute] exacerbation) or diagnosis 358.01 (Myasthenia gravis with [acute] exacerbation).
