Manage OASIS-C, Diagnosis Coding Pressure Ulcer Conflicts

Do you know when a grafted pressure ulcer becomes a surgical wound?

OASIS-C logic and ICD-9 coding logic don't always agree. That's certainly the case when it comes to pressure ulcer staging. When skin or muscle grafts are used to treat a pressure ulcer, you have to know when to consider the site a surgical wound and when it remains a pressure ulcer.

This handy chart from Trish Twombly, BSN, RN, HCS-D, CHCE, COS-C, director of coding with Foundation Management Services in Denton, Texas will help you remember how to make that call.
