Ob-Gyn Coding Alert

ICD-10:

Process How Your Polyp Codes Will Shift After October 1, 2015

For “mucous polyp of cervix,” report this ICD-10 code.

Polyps are small growths on the surface of the uterine wall or that might appear on the cervix, in the vaginal canal or even on the vulva that are easy to remove. But if the polyp is found in the uterus, don’t confuse these with fibroids, which are larger and usually imbedded in the uterine wall and require more work to remove.

ICD-9-CM Codes: Currently, you should report this condition with the following codes based on the documented site:

  • 621.0, Polyp of corpus uteri
  • 622.7, Mucous polyp of cervix
  • 623.7, Polyp of vagina
  • 624.6, Polyp of labia and vulva

ICD-10-CM Codes: Once your diagnosis codes shift, you should instead report one of the following ICD-10 codes:

  • N84.0, Polyp of corpus uteri
  • N84.1, Polyp of cervix uteri
  • N84.2, Polyp of vagina
  • N84.3, Polyp of vulva
  • N84.8, Polyp of other parts of female genital tract
  • N84.9, Polyp of female genital tract, unspecified

ICD-10-CM Change: Notice how ICD-10 groups these polyp codes of the female genital tract together. For the majority of these codes, you have a one-to-one correlation. However, 621.0 maps to not only N84.0 but N48.8 and N48.9.

Documentation: Terms you might see that represent polyp of corpus uteri (N84.0) are “polyp of endometrium” and “polyp of uterus NOS.”

You might also see “mucous polyp of cervix,” which had a specific code in ICD-9. In ICD-10, you should report N84.1.

Finally, if the provider reports “polyp of labia,” you should report polyp of vulva code N84.3.

Here’s how you’ll find these codes in the Alphabetic Index:

Coding Tips: Underneath the N84.- category, you’ll see an Excludes1 note that says you should not report any of these codes with adenomatous polyp (D28-) or placental polyp (O90.89).

Also, underneath code N84.0, you should be aware of an Excludes1 note that forbids you from reporting polypoid endometrial hyperplasia (N85.0-) at the same time as code N84.0.
