New J Codes for Contraception Will Change Billing

HCPCS has released two new J codes for contraceptive drugs that will change the way these drugs are billed. They take effect on Jan. 1, 2002.
 
J1056 is for injection, medroxyprogesterone acetate/  estradiol cypionate, 5mg/25 mg. The code is used for injections of the brand name Lunelle, a new contraceptive approved by the FDA for use in 2000. Prior to the release of this code, coders had to submit J3490 (unclassified drugs) when billing for Lunelle injections. 
 
The first injection of Lunelle is given within five days of the onset of a normal menstrual period and then every 28 to 30 days for as long as the patient remains on the drug. Providers report that Lunelle is a good option for patients who cant remember to take oral contraceptives every day or for whom other methods of birth control are  incompatible. The code for the injection procedure, which is submitted with the J code, is 90782 (therapeutic, prophylactic or diagnostic injection [specify material injected]; subcutaneous or intramuscular). The drug is typically injected into the patients thigh, hence the subcutaneous or intramuscular designation.
 
J7302 (levonorgestrel-releasing intrauterine contraceptive system, 52 mg) was introduced for the Merena contraceptive system. Merena is a new form of IUD that stores and releases the hormone Levonorgestrel. The  manufacturer, Berlex, states that the device imparts fewer of the negative side effects associated with traditional IUDs, such as heavier menstruations and painful periods, and can be useful in the administration of hormone replacement therapy (HRT).
 
Prior to the introduction of the code, coders submitted J3490 for the supply. The code for the procedure, 58300 (insertion of intrauterine device [IUD]), remains unchanged for the insertion.
