Here's Why the Scar Revision Location Matters

Question: The patient had the following surgery: Total Abdominal Hyster­ectomy (TAH), right salpingo-oophorectomy, left salpingectomy, and scar revision. How would I code the revision of the scar? 13100 series of codes?

Answer: If the area of the scar is within the incision location for the TAH, you will not get paid. Of course, you must have supporting medical necessity for revising the scar as well. That said, you should look for the code that most closely matches the work. The complicated repair code 13100 (Repair, complex, trunk …) series may not be it, but rather this could have been an intermediate repair instead (12001, Simple repair of superficial wounds of scalp, neck, axillae, external genitalia, trunk and/or extremities [including hands and feet] …).
