How to Report Removal of Tubal Remnants

Question: The patient is status post tubal ligation but later returned for laparoscopic cystectomy. The ob-gyn removed the remnants of the tubes.  Can we bill for the removal of the tubal remnants?

Answer: You should simply bill 58662 (Laparoscopy, surgical; with fulguration or excision of lesions of the ovary, pelvic viscera, or peritoneal surface by any method) for this surgery. You should not report 58661 (Laparoscopy, surgical; with removal of adnexal structures (partial or total oophorectomy and/or salpingectomy) in addition, since removing the remnants was incidental and this was not technically a salpingectomy.
